Please read carefully and if you have any questions, please ask. You have a 7 day period to withdraw from the Plan. If you withdraw, we will refund the payment made unless veterinary services have been provided under the Plan. If veterinary services have been provided, then full payment for these will be required.

  1. This is a 12 month contract. The minimum payment period for the Pet Health Care Discount Plan is 12 months. If the payments are stopped, for any reason, Harvest Veterinary Clinic is entitled to be paid for the losses. The starting point of these losses is the amount owing from the outstanding monthly payments due for the full 12 month period of the Pet Care Plan. (See 6. And 18. of Terms and Conditions on the reverse side of client copy of the Agreement Standing Order Form.)
  2. The Pet Care Plan and Agreement will automatically re-new after 12 months unless written notice is given 60 days or more prior to the anniversary of the date the Plan was started.
  3. This is not Pet Insurance. This is a scheme designed to spread the cost of veterinary preventative pet health care throughout a 12 month period at a discounted rate.
  4. The Pet Care Plan is non-transferrable and non-refundable. It is pet and client specific. The pet must be micro-chipped for identification purposes. Please note that this is a legal requirement from April 2016. The Pet Care Plan includes a micro-chip
  5. A primary course of vaccinations includes 2 vaccinations for a cat or a dog in accordance with the manufacturer’s instructions. If the second injection is late, a third injection will be required to ensure the pet is fully vaccinated and to keep the discount scheme operational. The cost of this extra injection will be £23.48 payable at the time it is given. It is the client’s responsibility to remember, book and attend a vaccination appointment prior to the deadline of the manufacturer’s directions.
  6. A booster vaccination must be given every 12 months in accordance with the manufacturer’s instructions. If the injection is late, a second injection will be required to ensure the pet is fully vaccinated and to keep the discount scheme operational. The cost of this extra injection will be £23.48 payable at the time it is given. It is the client’s responsibility to remember, book and attend a vaccination appointment prior to the deadline of the manufacturer’s directions.
  7. On the Pet Health Care Discount Plan there is a 6 month veterinary check included. This can be carried out 2 weeks either side of the due date. It is the client’s responsibility to remember, book and attend the 6 month check at the correct time. If this is not done in the required time frame, the 6 month check included with the Plan will be forfeited.
  8. On the Pet Health Care Discount Plan there is a senior pet (over 8 years) blood sample available at the 6 month veterinary check-up..
  9. On the Pet Health Care Discount Plan, de-fleaing and worming products may only be picked up at the 6 month check or at vaccination time. They may be picked up in place of the 6 month check which is then forfeited.
  10. For a puppy, the Plan will be started on an estimation of their adult size. If a dog enters a new weight band, or a dog does not reach the estimated weight band, then the monthly price will be changed accordingly.
  11. There is no upper age limit for registration.
  12. Pet Health Care Discount Plan vaccination in dogs covers Distemper, Parvovirus, Hepatitis, 4 forms of Leptospirosis, Parainfluenza Kennel Cough, Bordetella KC, and Infectious Canine Laryngotracheitis K C. (Not Rabies.)
  13. Vaccines For Life in dogs covers only Distemper, Parvovirus, Hepatitis and only 2 forms of Leptospirosis. Vaccines For Life in dogs do not cover Parainfluenza KC, Bordetella KC and Infectious Canine Laryngotracheitis K C.
  14. Pet Health Care Discount Plan for cats covers Feline Calicci Virus (flu), Feline Herpes Virus, Feline Enteritis, Feline Chlamydia and Feline Leukaemia Virus.
  15. Vaccines For Life in cats covers only Feline Calici Virus (Flu), Feline Enteritis and Feline Leukaemia Virus. Vaccines for Life in cats do not cover Feline Herpes Virus and Feline Chlamydia.
  16. The type of booster given is dependent on the data sheet produced by the manufacturer.
  17. The following treatments and services are excluded from the Pet care Plan: Laparoscopic (Key-hole) surgery, Hydrotherapy, On-line Prescriptions and Laser Therapy, Acupuncture, Homeopathy and Rabies Vaccines, and lungworm pathology testing..
  18. Harvest Veterinary Clinic reserves the right to refuse the right to join the scheme.
  19. Harvest Veterinary Clinic reserves the right to suspend the discount scheme if any monies are owed to the practice not on a pre-arranged payment scheme. This suspension may stand until all monies are paid in full.
  20. Harvest Veterinary Clinic reserves the right to expel any pet or client from the Pet Care Plan or the Clinic that does not follow all the Terms and Conditions of payment for the Clinic
  21. Harvest Veterinary Clinic reserves the right to change the Terms and Conditions. The up to date terms and conditions can be found at the surgery or on our web-site www.harvestvets.co.uk or on the reverse side of the Client copy of the Agreement Direct Debit Form.
